Yes you absolutely have to disclose. 49 CFR Part 40 and Part 382 require you to authorize release of your prior drug/alcohol violation history. If you hide it and they find out — and they will through the Clearinghouse — that's falsifying DOT records which is a much bigger problem than the original violation.
